Transcript Requests

Ankeny High School maintains copies of grades, attendance, and test results for any student who attended AHS or Northview Middle School during grades 9-12. We will be happy to mail or FAX a copy of your transcript to any college, place of business or your home address at no charge.

For a copy of your transcript please down load the transcript request form or submit a written request that includes:

Full name and/or name used in high school
Date of birth
Year of graduation
Student signature
Address of where you would like the transcript to be mailed.
